This survey has been designed to gather the views of short break providers. Your responses will help shape the service design and how we commission and develop short break services for children and young people with disabilities and their families.

The survey covers: how services currently operate, the potential for a provider framework, challenges you experience, rural and transport considerations, and the types of short breaks you offer.

All responses will be treated in confidence and used to shape future commissioning decisions. If you have any questions about this survey, please contact your local commissioner.

The survey is open until 5pm on Tuesday, 17 November 2026. It should take approximately 15 minutes to complete.
