Read the two different roles descriptions for the digital patient panel – lead patient influencer, and patient influencer, and decide which role you would like to apply for

This form will take about 20 minutes to complete. You can complete it over as many sessions as you need – just remember to use the same browser and device when you open the link again

Complete this application form by 4 September 2025 to be included in the first round of recruitment

If you are under 18, we will send a separate consent form by email for you to complete with your parent or guardian

For all applicants that apply by 4 September 2025 we will review applications and get back to you in September 2025
