BSW Together, the NHS integrated care system for Bath and North East Somerset, Swindon and Wiltshire is working towards improving how patients are discharged from hospitals. We would like to hear from people and their families who have recently experienced being discharged from a hospital to help inform the development of new resources that will support the hospital discharge process.
